Respite & Community Habilitation Caseworker

All Applicants Must Undergo Fingerprinting as per NYS-OPWDD

Community Habilitation (CH) Services provides independent living skills training, socialization skills, and integration into the community. CH assists individuals in acquiring, retaining, and improving adaptive skills and self-help skills needed to live independently at home and in community-based settings.

Participants and families partake in the development and implementation of the Individualized Habilitation plan. The plan focuses on the strengths, needs, and choices of the individuals in order to promote independence, individuality, inclusion, and productivity. Our professionally trained Community Habilitation Specialists assist the individuals on an array of outcomes to develop skills such as travel training, money management, writing, academics, etc.

  • Minimum Requirement Associates Degree.
  • Casework Preferred.
  • Detail Oriented, And able to travel within all five borough.
  • Monitoring, service provision and frequency insures families satisfaction with their DSP or services
